Overview:
POSITION SUMMARY:
The supervisor of the Health Home will manage and provide oversight to Care managers to assure delivery of care coordination services according to Health Home Policy and monitor services delivered to assure successful outcomes.
The Supervisor will also carry a caseload primarily of HARP clients.
The caseload will be dictated by recommendation of Lead Health Home and DOH guidelines
Salary: $21.
00 - $26.
00 per hour
Qualifications:
High School diploma/GED with additional training in human service delivery required; Associate’s, Bachelor’s or Master’s Degree in one of the following fields :Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
Degrees in other related areas may be considered.
Responsibilities:
Works as part of a Care Coordination team; attends and participates in team meetings to provide input and feedback around recipient’s medical, psychiatric, psychosocial conditions and co-morbidities.
Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources primarily of the Harp population.
Reviews clients’ medical, psychosocial functioning and status, updates plans and goals, review outcomes to further program goals; coordinates and monitors all care on behalf of recipient.
Maintains and coordinates effective and on-going communications with clients’, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other Agency staff.
Coordinates care planning and care transitions with other providers of services/ resources to ensure goal directed collaborative care.
Participates in the development/documentation /review and update of client centered comprehensive, integrated, care plan in consultation with other team members to ensure focus on desired outcomes and meet Core Health Home requirements.
Maintains documents, records, and other related reports in an organized, timely and accurate manner as per policy and procedure.
Utilizes Health Information Technology to communicate with Providers and provide services to Health Home Clients.
Acts as a resource/consultant to all team members on psychosocial, medical and/or substance abuse issues.
Provides telephonic as well as face to face outreach, engagement and service planning in the field.
Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services.
Promotes health and wellness activities for clients via individual and group sessions.
Facilitates service delivery, including making and assistance with keeping appointments; provides transportation as needed.
Participates in ongoing training, attends Provider meetings as required.
Assigns staff to attend meetings as needed and assures staff are meeting training requirements.
Serves as a mentor to provide guidance and support to other care managers.
Assists and or delegates in training of new employees.
Maintains monthly billing logs to ensure accurate billing to the Lead Health Home.
Evaluates appropriateness of assigned clients and assigns new clients to case managers, assures that caseloads are manageable and do not exceed DOH recommendations, Admits and discharges clients in to CRM to maintain accurate client data base.
Assures and oversees that staff completes documentation in a timely, organized and accurate manner as per policy and procedure.
Organizes and participates in required quarterly chart audits and makes recommendations and directs staff on corrective actions , submits documentation to lead health home as required
Develops and implements policies and procedures to reflect the standards of the health home as directed by the lead health home, implements revisions to policy and procedures as required.
Conducts weekly team meetings to review caseloads of case managers, identify any incidents, hallmark events and potential disengaged /discharges and assures timely follow up and completion submission of such reports as required.
Conducts employee evaluations, and counseling of employees as per United Methodist Homes policies and procedures
Assures accurate submission of employees Time cards
Monitors Weekend emergency calls
Demonstrates leadership, motivational direction and an ability to be a team player
Maintains communication with Executive Director
